Botox or Permanent Makeup First? The Correct Order (Brows, Lip Blush & Fillers Explained)

Many clients ask us:

Should I do Botox first… or my brows?
Do I get lip filler before lip blush?

It’s a very smart question — because facial treatments affect each other.

The order doesn’t just matter… it directly affects the final result.

Permanent makeup is designed based on anatomy. Injectables temporarily change anatomy.

So if we plan in the wrong sequence — the design may no longer sit where it was intended.

Let’s break it down simply.

First, we need to understand how muscle movement affects permanent makeup placement.

⸻

Botox or Permanent Makeup Brows — What Goes First?

At all times, Botox comes first.

What Botox Actually Does

Botox doesn’t fill or add volume.
Instead, it relaxes muscles.

Your eyebrows are not static — they constantly move based on:
• Forehead tension
• Muscle pull
• Natural asymmetry
• Resting facial expression
Because of this, even small muscle changes affect brow mapping.

As a result, when Botox relaxes those muscles, your brow position changes.
It can:
• Lift
• Drop
• Straighten
• Become more symmetrical
• Or change arch shape
For example, a relaxed forehead can slightly raise the tail of the brow.

⸻

Why This Matters for Permanent Makeup

We map brows using:
• your natural muscle movement
• resting expression
• brow lift strength
• forehead activity

In other words, if we tattoo brows before Botox, we are mapping on a moving target.

Therefore, after Botox settles, the brow can shift slightly — meaning the shape we carefully designed may no longer sit exactly where planned.
So, nothing is “ruined” — but it’s no longer optimal.
For this reason, we always want to design brows based on the final muscle position, not a temporary one.

⸻

Correct Order for Brows:

1. Botox
2. Wait about 2 weeks for full settling
3. Permanent makeup brows

This allows precise mapping and the most natural healed result.

⸻

Lip Filler and Lip Blush — Which Comes First?

Same principle.

Lip filler comes before lip blush.
This ensures the tattoo follows the final lip shape.

What Filler Changes

Filler affects structure, not just size:
• lip shape
• symmetry
• vermillion border
• projection
• balance between top and bottom lip

Because of this, lip blush is a tattoo placed along that border and shape.
If we tattoo first and then add filler — the skin stretches and pigment placement shifts.

That’s why we always tattoo the final lip — not the “before” lip.

⸻

Correct Order for Lips

1. Lip filler
2. Wait 2–4 weeks for swelling to fully settle
3. Lip blush

This produces the softest, most balanced healed result.

⸻

What If You Already Did Treatments in a Different Order?

Completely okay — we adjust the plan. However, timing becomes more important.
Depending on your situation we may:
• slightly modify placement
• recommend timing between procedures
• plan touch-ups differently

That’s why consultations are important, because every face has a unique muscle pattern.

⸻

Why Planning Order Matters

Permanent makeup is long-term.
Meanwhile, injectables are temporary.
So we always plan around the temporary treatments first and build the permanent design on top of that stable foundation.
Only then do we create the long-term design.

As a result, this prevents:
• asymmetry after Botox
• stretched lip pigment after filler
• unnecessary correction sessions

It’s not about trends. It’s about harmony.
Ultimately, proper sequencing protects your final result.

Botox & Fillers at Lash Boutique Cape Cod

Now, we offer medical aesthetics in Plymouth and Hyannis with Nurse Practitioner Cathy, RN, BSN.

Her philosophy:

Enhance what you already have.
Keep your expression.
Refine — never overdone.
For this reason, every treatment is planned conservatively and intentionally.

You can now coordinate:
• Brows
• Lip blush
• Botox
• Facial balancing

So everything can be done in one place, with treatments planned in the correct sequence.

If you’re unsure where to start, we’ll guide you through the order during your consultation.
